Localized lichen myxedematosus

簡介

在纖維母細胞(fibroblast)的疾患中，大概可以分成三個大類，分別是Fibrosis (fibroblast增生)、Sclerosis (製造過多collagen)、以及Mucinosis (製造過多mucin)。其中，lichen myxedematosus為同時合併了fibrosis和mucinosis的狀況，可以分成generalized和localized兩大類。本篇筆記主要整理localized lichen myxedematosus。

分類

1. 可以分成generalized和localized兩個類型
2. Generalized form：為scleromyxedema
3. Localized form：包括discrete papular lichen myxedematosus、acral persistent papular mucinosis、self-healing papular mucinosis、cutaneous mucinosis of infancy以及nodular lichen myxedematosus

Discrete popular lichen myxedematosus

1. 罕見的疾病，在男性較多
2. 臨床表現

2-5mm，雙側對稱，紅至紫色的丘疹

主要影響軀幹和四肢近端(不太會影響臉)

緩慢進展，但不會有系統性的侵犯

有些患者可能合併有HIV的感染

3. 預後良好，但是不太會自發性緩解

Acral persistent papular mucinosis

1. 蠟狀、皮膚色的丘疹，主要出現在手、手腕、前臂的伸側
2. 不會有系統性侵犯，也不會自發性緩解

Self-healing papular mucinosis

1. 罕見的類型，可以分成幼年型和成年型
2. 臨床表現基本上和discrete papular lichen myxedematosus相近

患者會急性出現許多丘疹和斑塊，可能合併系統性症狀(如發燒/關節痛)

3. 此狀況的特點為自發性緩解，時間約2個月到2年不等

Cutaneous mucinosis of infancy

1. 被認為是discrete papular lichen myxedematosus或acral persisitent papular mucinosis的兒童類型
2. 為紅色或膚色的丘疹，出現在軀幹和四肢，不會有系統性侵犯

Nodular lichen myxedematosus

1. 病灶為硬、膚色的結節(通常>6mm)，主要分布在軀幹和四肢
2. 個案很少，不會有系統性的侵犯
